It is very important you keep this appointment.  Due to the specialized nature of this appointment, significant time has been reserved for you.  If you have a scheduling conflict you cannot resolve, please contact your claims  examiner or attorney immediately. Changes to your appointment can be coordinated only thru your adjuster or attorney.  Our office does require 6 business days cancelation notice to avoid a late cancellation fee.  

If you need a language interpreter, please notify your claims examiner or attorney to arrange for one.

You will receive a questionnaire form our office. This questionnaire covers important information vital to Dr. Nissen. This may take some time to complete, and for this reason we are providing it to you several weeks in advance. Please make sure to bring it completed to your appointment with Dr. Nissen.
